Includes the applicant's letter of introduction, the medical test with answers given by the applicant and his certificate of physical capacity. Total of 7 1/2 pages, in ink. The letter of introduction reads: Naval Hospital, New York, Aug. 31, 1861. Gentleman: I was born at Little Falls, N.Y., March 12th, 1839 and am therefore twenty two years, and six months old. I have received my instruction in English & classical literature at Jefferson County Institute, Watertown, N.Y., at Lawrence Academy, Groton, Mass., & studied the French language at l'ecole Normole de Jaques Cartier at Montreal C.E. I attended one winter course of lectures at McGill Med. College, Montreal, two winter courses at Harvard Medical College, Boston, Mass., also one summer session of six months at the same institution where I graduated in July 1861. My medical instructor was Dr. H.G.P. Spencer of Watertown, N.Y. principally, but have received instructions from Dr. Amasa Trowbridge, formerly of the U.S. Army, also private instructions from the various assistant professors of Harvard University. Have studied medicine at these various sources four years. Have put up my own prescriptions while student at Deer Island Hospital and the Quarantine Hospital, Boston Harbor & gained the most of my practical knowledge of medicine & surgery at the same institutions together with the advantages of a country practice. Have read a limited amount upon zoology & batony, some months since have practiced taxidermy. Have studied Latin & Greek several years since & French more recently. My Post Office address is Watertown, Jefferson County, New York. Frederick B. Adams Lewis. Lewis was asked 8 medical questions on his test. This lot includes the questions as well as his detailed answers. Some of the questions were: What are the muscles of the forearm? Describe the process of the union of wounds. What is the process of the manufacture of calomel? What are the uses of Belladonna & its poisonous effects? What are the symptoms of imflammation of the membranes of the brain and the morbid appearances in fatal cases? Also includes his certificate of physical capacity which is dated at Brooklyn, Aug. 31, 1861. Light wear and age toning. Extremely interesting Civil War medical related documents! Doctor Frederick B.A. Lewis was appointed Assistant Surgeon, U.S. Navy, on Sept. 1, 1861, and served on the U.S.S. St. Louis and U.S.S. Mahopac during the Civil War.
