8 1/2 x 10 3/4, imprinted form filled out in ink. For Private Phillip J. Stevens, Co. C, 147th New York Infantry. He was killed on June 20, 1864, at Petersburg, Va., by a musket ball. The document gives his personal biographical information, muster dates, pay and bounty accounts. Light age toning. Fine document.
