The medal is in two pieces. The top section, backed with a pin, is decorated with a crouching eagle, wings spread, over the phrase "CENTENNIAL INAUGURATION", with a shield on the left with raised interior stars and stripes and the date "1789" in a diagonal band, and with a shield on the right with raised interior stars and stripes and the date "1889" in a diagonal band. The bottom section of the metal, linked to the top with a small metal ring, is shaped like a five-pointed star over a disc with lines radiating outwards. The rim of the star is decorated with the phrase "NEW YORK CITY APRIL 30TH 1889 FIRST IN WAR FIRST IN PEACE AND FIRST IN THE HEARTS OF HIS COUNTRYMEN". The interior of the star is decorated with thirteen raised stars surrounding a raised profile of George Washington facing left. The back of the lower piece bears the maker's inscription "C.G.
