I originally thought this was a 3 ½" x 1" x 1" metal 12-hole bullet mold. The widest part of each individual mold is .365" diameter. If you unscrew the knob, and swing the locking bar out of the way, the mold comes apart in three sections, hinged so that you can take the cast items out of the mold. See photos for more details. Looks to be brass and has wonderful old patina. I recently read w this is similar to a suppository mold from the Civil War era. If you know for sure, please let us know.
