Guaranteed to be old, original and authentic, this is the duplicate 1923 series form, issued by the Treasury Department / IRS for the purchase of Opium, Morphine, Coca Leaves, or Compounds, and shows approved by Congress December 17, 1914. Collectors Office shows Pridgen John L of San Antonio, Texas And it was authorized on August 25,1925 by Mr. James Bason. The front shows the controlled substances purchase information for the purchase of " Opium, Morphine, Coca Leaves, or Compounds, Manufactures, Salts, Derivatives or Preparations tof Under Section 2 of the Act of Congress, Approved December 17, 1914" The back of the form has the a full page (in small type) of information and instructions with all of the "ifs, ands, and buts" covering the purchase or controlled substances. It measures approximately 8 inches by 10 3/4 inches. Very solid and clean document with good margins. It has a few carbon-paper markings, and is in MINT - Unused CONDITION Scans show the front, the back, and both together. Great Reference material, and very frameable as a single, or in a group.
