This is a military issue C.A.T. (Combat Application Tourniquet) like the one issued in your Improved First Aid Kit (IFAK) or Field surgical kit. It is designed to be used with only the use of one arm, which is an excellent issue should it be need to put in on the opposite arm without further help.It is brand new and still in the bag!
