Herb Brooks signed 3x5 index card. I obtained this autograph at a minor league hockey game in 1999. Herbert Paul "Herb" Brooks, Jr. (August 5, 1937âe"August 11, 2003) was an American ice hockey coach , best known for coaching the U.S. hockey team to a gold medal at the 1980 Winter Olympics in an event known as the " Miracle on Ice ".
