This Original white form was to be forwarded by the Druggist to the office of the supervisor of permits..This was printed by U. S. Treasury Department, Bureau of Industrial Alcohol, and is a Prescription Form for Medicinal Liquor. This was issued under the authority of the National Prohibiton Act, as printed at bottom of Rx... When you hold this up to the light (pic #3) you can see INDUSTRIAL ALCOHOL" printed, watermarked in the paper, and the Treasury Seal in Latin in the center, and informative directions on back... The Medicine prescription is for 1 Pint WHISKEY , and directions are or look 1/2 TEASPOONFUL AS NEEDED (in Pharmaceutical lingo).....
