Historically interesting official Treasury Department forms of this type were discussed on the History Channel's production "Hooked". "United States Internal Revenue Order Form for Opium or Coca Leaves, or Compounds, Manufactures, Salts, Derivatives or Preparations Tof under Section 2 of the Act of Congress, Approved December 17, 1914" . This is the 1936 form , so stated, later version with watermark , printed on greenish buff paper, with decorative border and beautiful opium poppy flower design in red. The paper is watermarked "United States Internal Revenue Bureau. It measures 8-7/8 x 10-3/4". The left margin is wider than the right, as this is the duplicate order form and was stored in the order book after the original form was sent to the supplier. The exteme left margin has tiny perforations from the staples that once held the form in the book.The form was issued to B. P. Merrick, Vet., Berlin Heights, OH by the 10th District collector. The supplier was Abbott Labs, 111 N. Canal St., Chicago IL.We are offering (separately) several copies of this form, all used from July 1946 through Oct 1947. They are all in uniformly excellent condition. The specifics of the prescription differ slightly from form to form. Most are for 100 tablets of Morphine Sulphite 1/2 gram.CONDITION: Near Fine.
